GROUP DISCOUNT

Receive a 5% discount on main event registration when a minimum of five employees from the same organization register at the same time. Must submit by email, mail or fax with payment.

YOUR MAIN EVENT REGISTRATION FEE INCLUDES:

• Educational Sessions from Tuesday to Thursday
• Welcome Reception with Exhibitors on Monday
• Continental Breakfast Tuesday to Thursday
• Luncheon with Exhibitors on Tuesday and Wednesday
• Refreshment Breaks
• URL to Event Presentations Prior to the Conference
• Entrance to the Exhibit Hall
• Continuing Education Credits - Nursing, SPHP, PT, and OT
• Poster Session

Main event registration does not include pre-event workshops.

All registrants must pay in full before attending any sessions, events, or workshops. You can join online or complete the Registration Form and mail/fax, along with payment, to:

Please do not fax the Registration Form if payment is by check. Payment must accompany the Registration Form. Only registration with credit card payment can be faxed for processing. Registration confirmation will be sent via email only.

ONLINE REGISTRATION

Online registration closes at 11:50 pm Eastern on February 29, 2024. No registration will be accepted by email/mail/fax after this date. On-site registration starts at 7 am on March 4, 2024, at the registration area.

REFUNDS/CANCELLATION/REPLACEMENT POLICY

Refunds, less a 20% processing fee per registrant, will be granted for cancellations received in writing on or before February 5, 2024. No refunds will be made after this date for any reason, including the occurrence of any circumstance beyond the control of ASPHP (such as acts of God, war, or terrorism; natural disaster; or government regulations, strikes, civil disorder, or curtailment of transportation systems) to the extent that such circumstance makes it impossible for registrant(s) to attend. Registration substitutions may be made if requested in writing before March 1, 2024. ASPHP reserves the right to cancel/change any general session, pre- and post-event workshop, and breakout session. All applicable refunds will be issued following the closing of the event. Refunds will not be given for no-shows.

PRE-EVENT WORKSHOPS

ASPHP reserves the right to cancel any workshop if the minimum registration is not met. No meal service will be provided at any pre- and post-event workshops.

SPECIAL REQUESTS

If you are aware of any disability that might prevent you from participating in any part of this event, please contact ASPHP Headquarters at info@asphp.org or call 610-248-9911 so that we may work with you to make the necessary arrangements to allow your full participation. Please notify ASPHP Headquarters if you have any dietary restrictions.

HELPFUL INFORMATION

• Before you travel, plan ahead.
• Let the hotel know if you will be arriving late when you make your hotel reservation. If your flight is delayed, inform the hotel as soon as possible to secure your booking.
• Take your expensive jewelry/valuables with you or put them in the hotel safe. Don't leave them in your room.
• View the agenda ahead of time. Make sure you know when and where the sessions/events will be held and be prompt as a courtesy to the speaker and other attendees.
• Meeting rooms in the hotel are often too cold or too warm. Dress in layers and bring sweaters/shawls and socks with you.
• There are restaurants and shopping areas within walking distance from the hotel. Make sure you bring a pair of comfortable walking shoes.
• Remember to bring lots of business cards. You will need them for networking. • Relax, enjoy, and have fun!

By registering and attending the ASPHP 2024 National SPHM Education Event, attendees grant ASPHP the irrevocable right to use their likeness, voice, and any recordings captured during the event in photographs, digital video recordings, audiotapes, or any other audio or video media, in whole or in part, for marketing, educational, and general publicity purposes. This consent is given freely and voluntarily and may be used by ASPHP at any time in the future, in publications, on the ASPHP website, in social networking sites, videos, and promotional materials. Attendees waive their right to inspect or approve the finished product in which their likeness or recordings may be used and waive any right to compensation arising from or related to the use of this material. Attendees acknowledge that their name, photo, and details disclosing their identity may be revealed.

If an attendee does not want to be photographed or recorded, they must inform ASPHP Headquarters at info@asphp.org in writing prior to the start of the national event. Failure to do so will be interpreted as acceptance of these terms.
